Good Shepherd Integrated Healthcare Systems’ goal is to provide continuity of care to patients when they experienced health-care issues. By integrating Home Health Care Services to Outpatient Rehabilitation Treatment, the patient will be seen continuously by our health care professionals until the patient is nursed back to health.

The changing health care environment is more focused on a lesser stay at in-patient facilities. Patients are discharged to their homes to recuperate. However, patient care is still very much needed. That is the main focus of Good Shepherd. Our home health professional staff will take care of the patient at the home setting after the patient is discharged from the hospital following a procedure (cardiac, pulmonary, or orthopedic).

When the patient becomes stable or strong enough, the patient is immediately discharged to continue treatment at our outpatient facilities, whose approach to rehabilitation is more aggressive because the clinics have the proper types of equipment and monitors. There is continuity of care and monitoring of patients when the patients undergo rehab treatment at our facilities.

This approach to patient care reduces the incidence of re-admission to hospitals because patients are continuously cared for.

Good Shepherd Integrated Healthcare System (GSIHS) has been serving Nevada since 2006. It is our vision to provide comprehensive, quality patient-centered care that is focused on each patient’s unique needs. As time passed by, we have seen GSIHS grew fast and we have seen that continuum of care is very important to our patients/ clients.

We are referring to the continuum of care within our organization’s unit of collaboration from Outpatient Rehabilitation, Home Health, Hospice Care, and Personal Care Assistance. It is the transition from one unit to another — coordination, collaboration, and information transfer between different caregivers in different clinics, between the hospital, facilities, insurances that we partnered, and we are being recognized as a healthcare provider.

The continuum of care is important to our patients and caregivers/ family since it leads to an improvement in the satisfaction level, reduces costs, and improves health. Keeping up the continuum of care is especially significant for those patients who are more dependent on the health services, elderly patients, patients suffering from complex medical conditions, mentally vulnerable patients, and patients with chronic diseases.
